I, the undersigned, having enrolled my child/student in the EyeFive Youth Transportation and Road Safety event, and being fully aware of the risks of illness, injury and death, hereby save and hold harmless, the Douglas County Sheriff’s Office (DCSO), the Douglas County School District (DCSD), and Mountain Vista High School, as well as their instructors, volunteers, agents, or employees (collectively "Indemnitees") from any and all causes of action, suits, debts, damages, judgments, and demands whatsoever arising from my participation in this event, including any claims arising from the negligence of any of the Indemnitees. I further acknowledge that none of the Indemnitees will be held responsible for any illnesses, injuries or death that may be caused by or arising out of my participation in this EyeFive event. 

I acknowledge the contagious nature of COVID-19 and other infectious diseases and voluntarily assume the risk my child/student may be exposed to or infected by such diseases during the course and that such exposure or infection may result in personal injury, illness, disability or death.  I understand that there is a risk of my child/student being exposed to or infected by COVID-19 or other infectious diseases as a result of the actions, omissions, or negligence of myself and others, including, but not limited to, employees, volunteers, and EyeFive event participants and those with whom they interact.   

I affirm that my child/student is in good physical condition and does not suffer from any disabilities that would prevent, or limit, my participation in the EyeFive event and that, to the best of my knowledge, they have not been exposed to nor contracted COVID-19 or other infectious diseases, are not exhibiting any symptoms, and will withdraw my child/student if they begin exhibiting any symptoms or test positive even if they are symptom-free.  I agree my child/student will wear a helmet at all times while on bike during this course. 
Furthermore, I freely and voluntarily assume any and all risks known, or unknown, with my child/student that are inherent to being a participant in this EyeFive Youth Transportation and Road Safety event.
